The following is in agreement between Janet Nevins and _____________ for
childcare to be provided for ____________________________.
1. Childcare will be provided (check all that apply)

2. The hours of care will be _______ to _______.
3. Daycare fees will be made every Friday for the following weeks care.
- Newborn - 2yrs - $175 per week
- 2 yrs - 6 yrs - $125 per week
4. Parent may terminate this contract after giving a two-week notice to the provider.
5. Provider is paid for 2 weeks vacation and is also paid when the parent takes their vacation.
6. Provider is entitled to 5 paid sick days per year. (If I don't need them you will not be charged for them.)
7. The following are paid holidays:
- Good Friday & Easter Monday (April 6 & 9)
- Memorial Day (May 28, 2007)
- 4th of July
- Labor Day(September 3, 2007)
- Thanksgiving Day & Day after (Nov 22 &23, 2007)
- Christmas Eve, Christmas Day & Day after (Dec 24-26, 2007)
- New Years Eve & New Years Day ( Dec 31, 2007 & January 1, 2008)
8. If your child is absent due to sickness or any reason on your behalf, daycare fees still need to be
paid. If your child is absent due to my children or myself being sick then that day will count as a personal
day until the 5 days are used. Weekly payments are not based on attendance but on spaces available in the daycare.
